Sacral Iliac Pain

What is

The sacroiliac joint is the joint that joins the sacrum, the final bone of the spine in the form of a triangle, with the pelvis, the rest of the hip. This joint is responsible for allowing the transmission of forces from the lumbar spine to the hips and from there to the rest of the legs to the feet. Its function is essential to allow us to perform daily gestures such as getting up, walking, climbing stairs or running. Its pain can radiate to the gluteal region, end of the spine and back of the thigh.

Causes

This pain does not usually manifest itself on its own, but arises as a consequence of particular anatomical situations that lead to an overload of the joint:

  • Pregnancy and childbirth
  • Overloading after spinal surgery
  • Overloading due to disc and joint involvement in the lumbar region.
  • Scoliosis

Symptoms

This pain is located at the end of the lumbar spine, a little on the sides of the region at the end of the spine where it articulates with the pelvis. The pain worsens when sitting, squatting or going from standing to sitting.

Approach protocol

Diagnosis and assessment

A correct medical evaluation by means of a good anamnesis and physical examination will be necessary. The multidisciplinary vision is essential for a comprehensive, personalized approach, focused on achieving a prolonged relief of pain depending on the phase in which it is found.

Complementary tests

MRI and CT.

Treatment

  • Interventional medical treatment: the treatments are aimed at de-inflaming the joint and treating the nerves that pick up the sensitivity of the area, for this purpose the following options are available:
    • Sacroiliac joint blockage
    • Radiofrequency of the innervation of the sacroiliac joint.
    • Regenerative medicine treatments
      • Platelet-rich plasma: the compound extracted from the patient’s own blood has a concentrate of anti-inflammatory factors and cytokines that make it very useful for this pathology.
      • Cytokine-rich serum: this is a different process to platelet-rich plasma in which a compound rich in anti-inflammatory cytokines is also obtained from the blood.
      • Stem cells: originating from the patient’s bone or fat, they have a regenerative and anti-inflammatory power that makes them a unique treatment for pain relief.
  • Physiotherapy: methodology based on knowledge and clinical reasoning in which we combine a series of techniques and treatments both manual therapy and invasive techniques, always associated or complementing them with cutting-edge technology that allows us to accelerate the recovery process. Among other high-tech devices include Magnetolith, SIS, focal shock waves, Indiba, among others. This therapeutic combination allows us to activate the patient early to begin with a routine and a process of specific therapeutic exercises that allow us to enhance and strengthen the lost functional patterns.
  • Maintenance: after the treatment, the patient will be able to resume his usual activities, achieving a situation of less pain that will allow him to recover his muscle tone and improve his quality of life.
